진행된 의료진으로 진행된 안티에이징 시술 교육 ‘H.E.L.F ACT Livedemo’ 에서 석효현

들수록 인대와 지방층이 지지력을 잃고 아래로 쳐지면서 볼륨이 감소하고, 피부 표면에 주름과 굴곡이 형성된

다는 것이다. 석 원장은 “피부층을 3차원적으로 이해해야 비로소 효과적인 교정이 가능하다”고 설명했다.

이 같은 흐름 속에서 현장 의료진들의 관심은 자연스럽게 복합 시술 전략으로 이어진다.
PDO·PCL 계열의 의료용 실, HA필러, 스킨부스터

등 단일 시술보다 여러 제품을 조합해 구조와 피부 상태를 동시에 개선하려는 접근이 최근 안티에이징 트렌드로 자리 잡았다는 설명이다.

특히 필러 시장의 변화가 뚜렷하다. 과거에는 꺼진 부위에 많은 양의 필러를 채우는
방식이 일방적이었다면, 최근에는 적응 용량으로 피부 구조 지지를 도와주는 방향으로 트렌드가 이동하고

있다.

석 원장은 “이러한 ‘지지’ 개념에서 중요한 요소는 필러의 물성”이라고 설명했다. 유지인대를 받쳐주는 역할을 하려면 단단한 물성의 필러가 필요하다는 것이다.

대표적인 제품으로는 휴젤의 HA필러 ‘더채움’이 꼽힌다. 더채움은 시술 목적 및 부위에 맞춘 다양한 물성을 갖춘
제품이다. 특히 BDDE 등 가교제와 독성물질 제거를 위해

최대 15일간의 공정을 거치는 등 안전성을 높인 제품으로 평가받는다.

구조적 교정 이후에는 피부 표면의 질감과 탄력을 동시에 개선하기 위해 스킨부스터 시술이 병행된다. 스킨부스터는 진피 내 수분 유지 및 콜라겐 합성 촉진을 통해 전체적인 피부 상태를 끌어올리는 역할을 하며, 봉합사 실, 필러 시술과 시너지 효과를 낼 수 있다는 설명이다.

A New Treatment Approach for Addressing Structural Aging – All Liting Plastic Surgery, Seoul

“In the past, treatment focused on filling volume-deficient areas. Today, the y question is how to support and restore the facial structure itself.”

This was a key message emphasized by Dr. Seok Hyohyun of All Liting Plastic Surgery during the H.E.L.F ACT Live Demo, an anti-aging educational program held for medical professionals on the 21st. Discussions at the event extended beyond simple wrinkle correction and focused on the concept of structural aging—an approach that views aging not merely as a superficial skin concern but as a change occurring within the deeper soft tissue support structures of the face.

Using actual facial anatomical images, Dr. Seok explained age-related changes in retaining ligaments and facial fat compartments. As aging progresses, these supporting structures gradually lose strength and descend, leading to volume loss and the formation of wrinkles and contour irregularities on the skin surface.

Dr. Seok explained, “Only by understanding facial anatomy in a three-dimensional manner can truly effective correction be achieved.”

As this perspective evolves, medical professionals are increasingly interested in combination treatment strategies. Rather than relying on a single modality, current anti-aging trends emphasize combining PDO and PCL threads, HA fillers, and skin boosters to simultaneously improve structural support and skin quality.

The shift is particularly evident in the filler market. In the past, large-volume filler injections were commonly used to restore sunken areas. More recently, treatment approaches have moved toward using optimized volumes designed to support facial structures while maintaining natural outcomes.

Dr. Seok noted that one of the most important factors in this support-based concept is the rheological properties of fillers. To provide structural support for retaining ligaments, fillers with stronger lifting capacity and firmer characteristics are often required.

One representative example is Hugel’s HA filler, The Chaeum, which offers various rheological profiles tailored to different treatment indications and anatomical regions. The product is also recognized for its emphasis on safety, including an extended purification process of up to 15 days designed to remove residual cross-linking agents such as BDDE and other potential impurities.

Following structural correction, skin booster treatments are often incorporated to improve skin texture and elasticity. Skin boosters help enhance overall skin quality by improving hydration within the dermis and stimulating collagen production, creating synergistic effects when used alongside thread lifting and filler procedures.

The importance of individualized treatment design and long-term follow-up was also emphasized as key factors for achieving high patient satisfaction.

Dr. Seok explained, “Treatment planning should vary according to facial fat distribution, retaining ligament position and laxity, and skeletal structure. Personalized design, along with appropriate product and volume selection, is essential. Ongoing assessment after treatment and maintenance planning are equally important.”

Participants at the event also agreed that as anti-aging standards continue to evolve, expectations for physicians are changing as well.

Dr. Seok further emphasized, “Continuous physician education is essential to adapt to the rapidly evolving aesthetic landscape.”

As a board-certified plastic surgeon, Dr. Seok continues to lead educational initiatives and protocol development for aesthetic procedures based on a deep understanding of facial anatomy.

She added, “Anti-aging treatment is no longer about isolated improvements. It is evolving toward a more comprehensive approach that considers overall tissue environment and structural support.
